Application Form for Travel and Accommodation Support for US Participants of AIP III, 2005.

 

The conference organizers have just been informed by the US National Science Foundation that we will be getting a modest amount of funding for US participants of this conference. The primary goal is to support US researchers in their early to mid-career. Participants whose primary employment are in the US and who are either US citizen or non-citizen with a valid visa that permits travel and travel reimbursement will be eligible. The maximum amount of support is US$1000 per person, and can include travel and accommodation support.

 

Here are the steps to follow:

 

   1. Pick up (at the conference registration desk) and complete the travel reimbursement form from UCLA (the grantee).

Applicants should arrange to have the letters of recommendation sent via email to Bill de More at bill@math.ucla.edu.

 

   4. US Citizen/PR/US Visa status:

 

   5. Provide original receipts for those expenses applying for support for.

 

   6. The completed package (this form, the travel form, original receipts) should be mailed to Bill de More, Department of Mathematics, University of California at Los Angeles, Box 951555, Los Angeles, CA 90095-1555, USA.

 

   7. The deadline for receipt of the application package is July 30, 2005.

 

   8. Applicants will be informed of the award by Aug 30, 2005. Actual reimbursements should occur within one month afterwards.
